Health Records Notification

On or before December 1 of each year, the school district makes publicly available the immunization data they are required to submit to the State Board of Education by November 15th.

Immunization data made publicly available must be identical to the data the school district or school has reported to the State Board of Education.

In accordance with the Illinois School Code (105 ILCS 5/27-8.1), the District must annually make available the immunization data which must be reported to the Illinois State Board of Education. This information may be requested directly from the Illinois State Board of Education at (312) 814-9192.

PHYSICAL / IMMUNIZATIONS / DENTAL EXAMS / EYE EXAMS

Any student new to the state of Illinois will be required to have a complete immunization chart and physical and comprehensive eye exam on file.

Physicals/immunizations, dental exams, and comprehensive eye exams are required to be turned into the DeSoto Grade School main office, prior to the first day of school. Students who are not in compliance will not be allowed to start the current school year. Please see below for the requirements by grade level or for students new to the state of Illinois.

Forms may be picked up in the main office or a copy may be downloaded from the links on this page.

The office will be open during the summer for parents to turn in their exam forms.

GRADE LEVEL REQUIREMENTS:

New to Illinois - physical and comprehensive eye exam.

Kindergarten - physical/immunizations, dental exam, and comprehensive eye exam.

1st Grade - physical/immunizations (only required if the student has not attended Kindergarten)

2nd Grade - dental exam.

6th Grade - physical/immunizations and dental exam.
